Why we need your records
It may be necessary for the expert to review your medical history in order to complete their report. Consequently, Doctors Chambers or your representatives will ask that you complete and sign a Form of Authority (FOA) for the release of these records. Medical records are often requested to look for any pre-existing condition or details of treatment after your injury. Sometimes Solicitors ask us to obtain a medical report without review of records, but the doctor may request them after the examination.
