If you are to be seen by a GP, you will find a personal injury questionnaire enclosed with your letter - please complete this and give it to the expert at your examination. If you lose the questionnaire or make a mistake on it, you can download another copy here. Please remember to take 2 forms of identification (one with a recent photograph).
